We’re so glad you’re here! We want to make your visit as easy and comfortable as possible. Walk-ins are welcome Monday and Tuesday. Appointments are available Wednesday, Thursday, and Friday—please call our office to schedule your visit. We are closed Saturday and Sunday. We look forward to caring for you!

Your First Visit, Step by Step

Getting started is simple. Here's what to expect from scheduling through your first appointment.

Schedule Your Visit

Call our office or use our online appointment request form to book your first visit. Let us know if you have any special concerns or needs when you call.

Complete Your Paperwork

Download and complete new patient forms before your visit to save time. Bring the completed forms with you or ask about completing them electronically.

Arrive Prepared

Plan to arrive 15 minutes early for your first visit. Bring your insurance card, a valid photo ID, and a list of any current medications.

Meet Your Provider

Your provider will take time to get to know you, review your health history, and work with you to build a personalized care plan.

What to Bring

To make the most of your first appointment, please bring the following items with you.

  • Valid government-issued photo ID (driver's license, passport, etc.)
  • Current insurance card(s) (if needed for referral or other insurance covered services
  • List of all current medications, vitamins, and supplements (including dosages)
  • Names and contact information for any specialists you currently see
  • Relevant medical records, lab results, or imaging from previous providers
  • Completed new patient forms (if downloaded in advance)
  • Payment method for any applicable co-pays or fees

Forms are currently available at our office. Please call ahead if you would like them emailed to you before your visit.

Office Policies

Understanding our office policies helps ensure every patient receives the best possible care.

Appointment Cancellations

We ask that you provide at least 24 hours notice if you need to cancel or reschedule your appointment. This allows us to offer that time to another patient in need.

Late Arrivals

If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure all patients receive the time and attention they deserve.

After-Hours Care

For urgent medical concerns after office hours, please call our main number for after-hours guidance. For life-threatening emergencies, call 911 or go to your nearest emergency room.

Prescription Refills

Prescription refill requests require an up-to-date visit on file. Please allow 48–72 business hours for refill processing. Controlled substances require an in-person visit.

Medical Emergency? If you are experiencing a life-threatening emergency, call 911 immediately or go to your nearest emergency room. Do not wait for an office appointment.
